Each of the primary care and specialty providers listed in "Optimum Provider Management Group, LLC's Network of Providers" has entered into participation agreements with Optimum Provider Management Group, LLC, under which the providers agree to participate in the ACO and are eligible to receive a portion of any savings generated. These agreements define Optimum Provider Management Group, LLC's and the participating providers' roles and obligations in providing high-quality, coordinated care and improving our patients' overall health. All privleges are waived in order to be completely transparent in line with the agreement with the above and the results provided to ensure quality services are rendered. 

For medical professionals, our services are in great demand. Thousands of our providers are located across the United States, including Alaska and Hawaii. We are proud to have certified a significant number of these suppliers to date.
